NOTARY PUBLIC - Post Commission Instructions
UPON RECEIPT OF YOUR COMMISSION.. The Oath of Office must be taken in the presence of a Notary Public who will notarize your signature. A copy of the completed Oath of Office must be returned to the Secretary of State, Notary Public Section preferably via email, or alternatively by mail or fax (contact information below). There is no fee charged for this recordation. IMPORTANT: A Notary Public may not perform any notarial acts until the Oath of Office is received by the Secretary of State, Notary Public Section. A confirmation of receipt will be sent back to the Notary Public.

NAME CHANGE OPTIONS … If your name changes during your appointed notary public term, you have two options:
Continue to sign your name as it appears on your seal AND your new name each time you notarize, or
Provide a written request for a name change certificate, submitting a copy of the document showing the change (e.g. marriage certificate, divorce decree, etc.) and a $10.00 fee.
Reminder: Always sign your name as it appears on your commission.
SEAL/RUBBER STAMP … To obtain a notary public embossed seal or rubber stamp, you may contact any office supplier. They may require you to show proof of appointment (have your letter and commission available).
LOST COMMISSION … If you lose your commission, you can obtain a duplicate by submitting a written request to the Notary Public Section (address above) with the following information:
- Your name as it appears on your commission
- Birth date
- Your current address and phone number
There is a $10.00 fee for this service.
